Findings of investigation into COVID-19 active cases privacy breach
Mr Michael Heron, QC, led a State Services Commission investigation into a breach of privacy regarding sensitive personal information. Deputy State Services Commissioner Helene Quilter announced the findings on 30 July 2020. The investigation looked into who or what caused the disclosure of the information, and what might have prevented the information from being disclosed and what, if any, improvements might prevent that happening again in the future. The investigation found that sensitive personal information was passed to someone who was not authorised to see it, who then placed it in the public arena.

Report on the Inquiry into the Treasury’s Budget related Information Security Systems
Ms Jenn Bestwick undertook an inquiry into the Treasury’s Budget related information security systems. Ms Bestwick reported to the State Services Commissioner who released the Inquiry Report.

Report on the Investigation into actions and statements of Mr Makhlouf
The Deputy State Services Commissioner undertook an investigation into Mr Makhlouf’s actions and public statements about the causes of access to Budget-sensitive material. The Deputy State Services Commissioner reported to the State Services Commissioner who released the Investigation Report.
Report on Investigation into the conduct and behaviour of the Retirement Commissioner, Ms Diane Maxwell
At the request of the Minister of Commerce and Consumer Affairs the State Services Commissioner undertook an investigation into allegations of bullying by the Retirement Commissioner, Ms Diane Maxwell. The Commissioner reported to the Minister who released the Investigation Report.
Review into how agencies handled complaints of inappropriate behaviour
The State Services Commission undertook a review into how the Ministry of Justice and Department of Corrections handled complaints of inappropriate workplace behaviour in 2016. The Commissioner has accepted all of the recommendations.
External security consultants’ inquiry findings
A State Services Commission investigation into the use of external security consultants by government agencies has uncovered failings across the public service, including breaches of the code of conduct. The Commission paid the Inquirers $350,000 to conduct the nine month investigation.
Counties Manukau DHB Review: Acting Director-General of Health

The State Services Commission conducted a review into questions raised in Parliament by Jami-Lee Ross MP.
It is the Commission’s usual practice to release in full all documents relating to reviews and inquiries – documents that would otherwise be available under the Official Information Act.
